Is it possible to create a multi-field 'input' msgbox?

First Name
Last Name
Email Address
Mailing Address
Phone

...etc.? All in the same msgbox. Assigning different variable of course.

Menu "Auto complete".

"Send" option --> "Automation" --> search for "Message box" ... and fill in as you like.

My apologies, I left the word 'input' out of the question. I need multiple input fields within the same form/msgbox

Thanks marcelolima! I understood what you was saying... I just didn't know how to go about the different type of inputs sliders etc.

I found an example of what I'm looking for it's the Medical HPI Example
